Job summary

Solomon Medical is seeking a full-time physician to cover SNFs in San Jose, CA and Mountain View, CA. The role includes post-acute and long-term care, with potential for part-time arrangements.

You will work with a Nurse Practitioner to develop and coordinate comprehensive care plans and ensure compliance with Medicare guidelines. The successful candidate will have BC/BE in a primary care specialty, California license, and experience in EMR documentation and billing.

Qualifications

• BC/BE primary care specialty and DEA
• California state license required
• Comprehensive knowledge of acute and long-term care treatment modalities
• Strong leadership and communication skills
• Experience with EMR documentation and billing/coding systems

Responsibilities

• Provide medical management and oversight of acute and long-term treatment plans
• Deliver direct patient care for new admissions and routine follow-ups per Medicare guidelines
• Coordinate treatment plans with facility staff and NP/clinical leadership
• Ensure compliance with state/local regulations and Solomon Medical protocols
• Document thoroughly and manage billing per Solomon Medical guidelines
• Collaborate with the Medical Director to align care models with facility protocols
