Job summary

Solomon Medical Inc. is seeking a physician for full-time coverage at SNFs in San Jose and Mountain View, CA. The role may be part-time for the right candidate, with a flexible schedule and Monday–Friday workdays.

The physician will work with a Nurse Practitioner and Medical Coordinator to deliver comprehensive care plans for post-acute and long-term residents, ensuring regulatory compliance and high-quality patient care.

Qualifications

BC/BE Primary Care with California license; DEA; DHEC credentials.
Comprehensive knowledge of acute and long-term care.
Leadership, communication and organizational skills.
Experience with EMR and billing/coding processes.

Responsibilities

Provide medical management and oversight of acute and long-term treatment plans.
Direct patient care for new admissions and routine follow-ups per Medicare guidelines.
Coordinate care with NP and Medical Coordinator.
Ensure compliance with state and local regulatory requirements.
Complete competency and advanced care plans for new patients.
Documentation and billing with EMR/E/M coding.
Coordinate treatment with facility staff and leadership.
Align with Medical Director to meet facility protocols.
