Qualifications

• Must possess a valid and current state Physical Therapy license.
• Ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide outpatient physical therapy to residents on-site.
• Implement residents’ personal care plans with attention to preferences and routines.
• Collaborate with interdisciplinary clinical team to address residents’ needs.
• Contribute to a holistic approach supporting physical, spiritual, emotional, and clinical well-being.
