Job summary

Brooksby Village by Erickson Senior Living is seeking a Per Diem on-site Out-patient Physical Therapist to support residents across a continuum of care on our campus in Peabody, MA. You will implement personal care plans and work with a cross-disciplinary team to promote mobility, safety, and quality of life for older adults.

The role requires a current PT license, experience with older adults, and accurate documentation for reimbursement.

Qualifications

• Valid state Physical Therapy license.
• Experience with older adult populations preferred.
• Ability to complete documentation for reimbursement and regulatory requirements.

Responsibilities

• Implement residents' personal care plans based on their goals and needs.
• Collaborate with interdisciplinary clinical team to address residents' care.
• Provide on-site outpatient PT services to Brooksby Village residents.
