Brooksby Village by Erickson Senior Living is seeking a Per Diem on-site Out-patient Physical Therapist to support residents on our campus in Peabody, MA. You will implement personal care plans, promoting mobility and functional independence within a holistic, longitudinal care model.

This role offers an on-site setting with a diverse interdisciplinary team, access to wellness resources, and a competitive hourly pay range.

Formación

• Must possess a valid state Physical Therapy license.
• Ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsabilidades

• Provide outpatient physical therapy to residents on-site.
• Implement individualized care plans focusing on residents' strengths and preferences.
• Collaborate with interdisciplinary team to address residents' needs and goals.
• Support residents' physical, emotional, and clinical well-being throughout care.
