You will implement personal care plans, support residents’ functional goals, and collaborate with the interdisciplinary team to address physical, emotional, and clinical needs.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
