Job summary

Brooksby Village by Erickson Senior Living is seeking a Per Diem on-site Out-patient Physical Therapist to join our team in Peabody, MA. You will work within our campus community, supporting residents’ physical, emotional, and clinical needs and helping implement personalized care plans.

The role requires a current MA PT license, documentation skills for reimbursement and regulatory compliance, and experience with older adult populations.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Degree and training must meet current Medicare and state requirements.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide holistic physical therapy care to residents, focusing on their strengths, preferences, and routines.
• Implement residents’ personal care plans as part of an interdisciplinary team.
• Collaborate with physicians, therapists, and staff to address residents’ clinical needs and support their wellbeing.
