Job summary

Pinnacle Fertility is seeking a Patient Navigator to guide fertility patients through their treatment journeys in a fast-paced, onsite role in Mount Pleasant, SC. You will coordinate appointments, educate patients on procedures, and collaborate with clinicians to ensure seamless care.

The ideal candidate has at least 2 years in a clinical setting, strong knowledge of insurance processes and patient advocacy, excellent communication, and EMR proficiency. Bilingual skills are a plus.

Qualifications

• Minimum of 2 years of experience in a clinical healthcare setting, including patient care coordination or case management.
• Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency in using electronic medical records (EMR) and other healthcare software.
• Bilingual skills are a plus.

Responsibilities

• Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
• Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
• Assist patients with understanding their medical care, insurance coverage, and available financial resources.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
• Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the EMR system.
• Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
• Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
• Educate patients on treatment protocols, self-care strategies, and medication usage.
• Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
• Advocate for patients’ needs and help resolve any issues or barriers to care.
• Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
