Job summary

UnitedHealth Group is seeking an Onsite Care Coordinator to optimize patient recovery journeys and coordinate discharge planning with the PAC inter-disciplinary team. The ideal candidate holds an active clinical license (RN, PT, OT, or SLP), has 5+ years of clinical experience, a valid driver’s license, and can work within the specified local radius.

Qualifications

• Active, unrestricted registered clinical license required in state of hire as RN, PT, OT, or SLP.
• Candidate must reside within or near the county listed on the job description and be within 30 miles of the site.
• 5+ years of clinical experience and valid driver's license with reliable transportation.

Responsibilities

• Engage with patients, families, or caregivers weekly on-site or telephonically to support discharge goals.
• Collaborate with interdisciplinary teams and participate in meetings to review discharge plans.
• Perform SNF assessments using CMS criteria and monitor continued stay approvals.
• Document accurately in Coordinate and maintain HIPAA-compliant practices.
• Adhere to local, state, and federal policies and promote a positive team environment.
