Qualifications

• Active, unrestricted clinical license in the state of hire (RN, PT, OT, or SLP).
• Minimum 5+ years of clinical experience.
• Driver's license and reliable transportation.

Responsibilities

• Engage with patients, families, or caregivers weekly or on-site to support discharge goals.
• Collaborate with care teams in interdisciplinary meetings.
• Review discharge plans with providers and families.
• Perform SNF assessments and CMS-based criteria checks.
• Maintain timely, accurate documentation in Coordinate.
• Adhere to HIPAA and local regulations.
