Qualifications

• Minimum 5 years of clinical experience.

Responsibilities

• Engage with patients and families weekly, on-site or telephonically, to support discharge goals.
• Collaborate with interdisciplinary care teams and participate in meetings.
• Review discharge plans with providers and families against CMS criteria.
• Perform SNF assessments and continued stay reviews using CMS criteria.
• Enter timely documentation and maintain HIPAA/compliance.
