Job summary

Optum Home & Community Care is seeking a Care Coordinator, Onsite to optimize patients' recovery journeys. You will coordinate discharge planning, engage patients and families, and work with interdisciplinary care teams to ensure timely transitions of care.

The role requires an active clinical license (RN/PT/OT/SLP) and 5+ years of experience, with on-site duties within a 30‑mile radius of your home. Strong CMS knowledge and MS Office proficiency are preferred.

Qualifications

• Active, unrestricted clinical license in the state of hire (RN, PT, OT, or SLP).
• Minimum 5+ years of clinical experience.
• Shall support onsite facility needs within a 30‑mile radius of home location.

Responsibilities

• Engage with patients, families or caregivers weekly on-site or telephonically to support discharge goals.
• Collaborate with care teams during interdisciplinary meetings.
• Review discharge plans with providers and families.
• Perform SNF assessments using CMS criteria.
• Coordinate SNF stays and ongoing reviews and authorizations.
