Job summary

Optum is seeking a Care Coordinator- Onsite to optimize patient discharge planning and SNF transitions. The role works with an inter-disciplinary team to ensure timely, quality transitions and strong patient engagement.

The position is hybrid with three in-office days weekly, requiring 5+ years of clinical experience and an unrestricted license in the hire state. Strong communication and organizational skills are essential.

Qualifications

• Active, unrestricted licensed in state of hire (RN, PT, OT, or SLP).
• 5+ years of clinical experience.
• Support on-site facility needs within 30 miles of home location (manager discretion).
• Residence within or near the county listed on the job description.
• Driver's License and reliable transportation.

Responsibilities

• Coordinate discharge planning with the inter-disciplinary care team.
• Perform SNF assessments using CMS criteria on admission and throughout stay.
• Review discharge plans with providers and families.
• Complete SNF concurrent reviews and update authorizations timely.
• Engage with patients, families, and caregivers on-site or by phone weekly.
• Attend care conferences and ensure appropriate referrals to Health Plan/Case Manager.
