Job summary

Optum Home & Community Care is seeking a Care Coordinator, Onsite, to optimize patients’ recovery journeys and coordinate discharge planning across the post-acute care network. You will engage patients and families, work with interdisciplinary teams, and ensure timely SNF assessments and approvals.

The role requires an active clinical license (RN/PT/OT/SLP), 5+ years of clinical experience, and a driving license for local travel. On-site facilities within 30 miles of your home will be supported.

Qualifications

• Active, unrestricted clinical license required in state of hire – RN, PT, OT, or SLP.
• Candidate must support on-site facility needs within 30 miles of home location.
• 5+ years of clinical experience.
• Reside within or near the county listed on the job description.
• Driver’s license and reliable transportation.

Responsibilities

• Engage with patients, families, or caregivers weekly or as needed on-site or telephonically.
• Collaborate with health care teams and participate in interdisciplinary meetings.
• Review target outcomes and discharge plans with providers and families.
• Perform SNF assessments using CMS criteria on admission and during stay.
• Assess continued suitability for SNF setting according to CMS criteria.
• Complete SNF continued stay reviews and update authorizations timely.
• Review referral requests for continued stay when delegated for utilization management.
• Maintain knowledge of multiple contracts/clients to cover needs.
• Manage caseload efficiently with good time management.
• Enter accurate documentation into Coordinate system.
• Adhere to policies and maintain PHI confidentiality.
