Job summary

OptumCareers is seeking a Care Coordinator- Onsite to optimize patient recovery journeys with SNF assessments and discharge planning. The role engages patients and families to share information and facilitates informed decisions while linking them to the appropriate health care staff.

The position follows a hybrid schedule with four in-office days per week, demanding strong collaboration with physicians, social workers, and therapists to ensure smooth transitions of care.

Qualifications

• Active, unrestricted clinical license in state of hire (RN/PT/OT/SLP).
• 5+ years of clinical experience.
• Ability to support on-site facility needs within 30 miles.

Responsibilities

• Coordinate discharge planning with the inter-disciplinary care team.
• Engage patients and families to share information for informed decisions.
• Review CMS criteria for SNF admissions and ongoing stays.
• Complete SNF concurrent reviews and update authorizations timely.
• Attend SNF rounds and provide up-to-date information to leadership.
• Maintain confidentiality of PHI and adhere to HIPAA regulations.
