Qualifications

• Active, unrestricted clinical license in RN, PT, OT or SLP in the hire state.
• Minimum of 5 years of clinical experience.
• Driver's license and reliable transportation required.

Responsibilities

• Serve as liaison between patients and care team to ensure smooth transitions.
• Perform SNF assessments using CMS criteria at admission and during stay.
• Review discharge plans with providers and families.
• Enter timely, accurate documentation; monitor census and barriers.
• Attend SNF rounds and care conferences; coordinate referrals as needed.
