Job summary

Optum Home & Community Care, part of the UnitedHealth Group family, seeks a Care Coordinator, Onsite to support discharge planning and SNF coordination. This role engages patients and families, coordinates with interdisciplinary teams, and ensures timely transitions of care at our Los Angeles area facilities.

The successful candidate will hold an active unrestricted clinical license (RN, PT, OT, or SLP), have 5+ years of clinical experience, and be able to travel within a 30-mile radius.

Qualifications

• Active, unrestricted clinical license in state of hire (RN, PT, OT, or SLP).
• Minimum 5 years of clinical experience.
• Willingness to support onsite facility needs within a 30-mile radius.

Responsibilities

• Engage with patients, families, or caregivers weekly or on-site to support discharge goals.
• Collaborate with care teams during interdisciplinary meetings.
• Review discharge plans with providers and families.
• Perform SNF assessments using CMS criteria and monitor patient status.
• Maintain timely documentation and adherence to policies.
