Job summary

The ideal candidate has an unrestricted RN license in Kentucky, at least 3 years of acute care experience, and proficiency with EMR and MS Office. Travel within commute distance and strong collaboration skills are valued.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficient in Microsoft Office and electronic medical records.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Lead discharge planning and coordinate with primary care providers.
• Identify gaps in care and develop strategies to address them.
• Collaborate with providers and care teams for timely, coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
