Humana is seeking an Onsite Care Coordination Nurse to support members through critical care moments. You will perform face-to-face assessments, lead discharge planning, and collaborate with care teams to ensure safe transitions from hospital to home.

The role combines in-person care at assigned facilities with remote coordination tasks, requiring 3+ years acute care experience and Kentucky RN licensure. Hybrid schedule, driving distance to Lexington, KY preferred.

Formación

• Current, unrestricted RN license in Kentucky.
• Must reside within 30–60 minutes driving distance to St Joseph Hospital, Lexington.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office and EMR/EHR systems.
• Valid driver's license and own vehicle insurance.

Responsabilidades

• Conduct comprehensive face-to-face assessments for newly admitted members in acute care facilities.
• Lead discharge planning from admission through post-discharge, coordinating with PCPs.
• Identify gaps in care and develop strategies to address safety and discharge barriers.
• Collaborate with providers, case managers, and healthcare teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control and patient privacy standards at all times.
