Job summary

Humana is hiring an Onsite Care Coordination Nurse to support members during critical moments in their care journey. You will conduct face-to-face assessments, lead care coordination efforts from hospital to home, and identify gaps in care to improve outcomes.

This hybrid role combines on-site engagement at facilities with remote coordination and documentation. The position requires RN licensure in Kentucky, 3 years of acute care experience, and strong discharge planning skills.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Must be located within 30–60 minute driving distance to CommonSpirit‑ St Joseph Hospital from home base.
• Minimum of 3 years recent acute care nursing experience (medical-surgical, critical care, etc).
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office applications and electronic medical records.
• Valid driver’s license, dependable automobile, and proof of personal vehicle liability insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Support and educate members on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; develop strategies to address issues.
• Collaborate with providers, case managers, and healthcare professionals for coordinated care and discharge planning.
• Provide education on diagnoses, medications, and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control and patient privacy standards.
