Qualifications

• Current, unrestricted RN license in Kentucky.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office applications and electronic medical records.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members in acute care facilities.
• Support discharge planning from admission through post-discharge and coordinate follow-up visits.
• Identify gaps in care, safety concerns, and barriers to discharge; implement solutions.
• Collaborate with providers, case managers, and health teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures; document promptly.
• Maintain infection control, safety, and patient privacy standards; complete required documentation.
