Humana is seeking an Onsite Care Coordination Nurse in Lexington, KY. The role combines in-person assessments with hybrid remote coordination, focused on safe hospital-to-home transitions for newly admitted members.

You will identify gaps in care, educate patients and families, and collaborate with clinicians to ensure timely discharge planning. The position requires an RN license in Kentucky and at least 3 years of acute care experience; vaccination and background checks may apply.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Valid driver's license and private vehicle.
• Hybrid or onsite work arrangement.

Responsabilités

• Conduct comprehensive face-to-face assessments for newly admitted members in acute care settings.
• Lead discharge planning from admission through post-discharge and coordinate follow-ups.
• Identify gaps in care and barriers to discharge; develop practical strategies.
• Collaborate with providers and care teams for timely, coordinated care and discharge planning.
• Educate members on diagnoses, medications, and preventive measures to reduce readmissions.
• Complete documentation accurately and maintain privacy standards.
