Qualifications

• Experience with Microsoft Office and electronic medical records.
• Valid driver’s license and reliable vehicle.
• Must live within 30–60 minutes of St Joseph Hospital in Lexington, KY.

Responsibilities

• Conduct comprehensive, face-to-face assessments of newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge with follow-up visits.
• Identify gaps in care and barriers to discharge; implement improvement strategies.
• Collaborate with attending providers and care teams for timely, coordinated discharge planning.
• Educate members on diagnoses, medications, and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety, and patient privacy standards.
