Humana Inc. is seeking an Onsite Care Coordination Nurse to support members at affiliated facilities in Lexington, Frankfort, Versailles, Nicholasville and surrounding areas.

The role combines in-person assessments with hybrid remote coordination to ensure safe hospital-to-home transitions. The ideal candidate is an experienced RN with Kentucky license, strong discharge planning and care coordination skills, and the ability to work independently within a collaborative care team.

Qualifications

• Current, unrestricted RN license in Kentucky.
• 3+ years acute care nursing experience (e.g., medical-surgical, critical care).
• Experience with discharge planning or care coordination in hospital or health-plan settings.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities to evaluate needs and plan care.
• Lead discharge planning and coordinate follow-up with primary care providers.
• Identify gaps in care and barriers to discharge; implement mitigation strategies.
• Collaborate with providers and care teams to ensure timely, coordinated care and documentation.
