Job summary

The Cancer & Hematology Centers in Michigan is seeking a Medical Social Worker to provide psychosocial care for patients receiving cancer treatment. You will perform biopsychosocial assessments and coordinate care plans to address patients’ psychosocial needs.

You will facilitate advance care planning and assist with advance directives, while offering emotional support and guiding patients through financial screening and assistance programs.

Qualifications

• Master’s degree in Social Work is required.
• Full LMSW licensure is required.
• Recent clinical experience in medical/oncology or Medicaid/Social Services preferred.

Responsibilities

• Completes biosychosocial assessments on all patients receiving treatment at The Cancer and Hematology Centers.
• Makes referrals to appropriate support organizations based on findings.
• Follows up on distress screenings, providing appropriate emotional support and referring to ancillary resources as needed.
• Facilitates conversations surrounding advance care planning and advance directives.
• Assists patients with applications for patient assistance programs and financial options.
• Assists patients in navigating insurance options when coverage is lost or patients are uninsured.
• Serves as a patient advocate to access federal, state, and local resources.
• Documents financial assessments, interventions, outcomes, referrals, and services provided.
• Collaborates with pharmacy, reimbursement, and nursing departments to coordinate services.
