Job summary

The Cancer & Hematology Centers in Michigan seeks a Nurse Navigator to guide patients through the cancer care continuum, providing patient-centered education, triage, and coordinated care across infusion, imaging, surgery, and genetics services.

This role requires an RN licensed in Michigan with oncology certification or plans to obtain it within 12 months, BLS, MICMT training or equivalent, and strong communication and care-coordination skills to support timely treatment.

Qualifications

• Licensed Registered Nurse in Michigan.
• Oncology Certified Nurse or plan to obtain within 1 year.
• ONS Chemotherapy/Biotherapy Provider Card or within 6 months.
• MICMT Training or acquisition within 60 days.
• BLS certification or acquisition within 90 days.
• Successful completion of orientation program.

Responsibilities

• Function as primary clinical contact for patients and families at consultation and during transition to treatment.
• Coordinate diagnostic work-up, treatment starts and follow-up visits across ambulatory oncology services.
• Collaborate with physicians, infusion nurses, pharmacists, social workers, and care teams.
• Provide patient education on diagnosis, treatment options, and clinic workflows.
• Support timely authorizations, referrals, and tests to avoid delays in care.
• Identify barriers to treatment and assist with transportation, finances, and scheduling.
• Track patient progress and document navigation interventions in the EHR.
