Fairview Health Services in Minnesota is seeking a float oncology pharmacist to provide clinical support in a complex retail pharmacy operation. You will partner with the care team to drive medication safety, formulary programs, and regulatory compliance while educating students, residents, and staff.

This float role rotates through Adult Fairview Ambulatory Infusion Centers (Clinics, Surgery Center, Ridges, Maple Grove, Southdale) with Clinics and Surgery Center as home site.

Formación

• 1 year pharmacist experience in complex pharmacy operation or completion of an ASHP-accredited PGY1 residency program.
• Licensed or eligible for licensure as a pharmacist in Minnesota.

Responsabilidades

• Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
• Act as resource for pharmaceutical care to patients including ensuring appropriateness of medication orders, drug selection, formulary accuracy and compliance with initiatives.
• Provide clinical pharmacy services to all patients in assigned area including daily reviews, monitoring, and documenting changes per policies.
• Distribute and/or prepare medications, sterile products, IV mixtures and other orders as needed.
• Educate patients, staff and students on drug therapies.
• Execute the function of staff pharmacist.
• Other duties as assigned.
