Job summary

Fairview Health Services is seeking a Hematology/Oncology Pharmacist based at Southdale Cancer Center to deliver comprehensive clinical pharmacy services within a complex infusion setting, focusing on oncology and specialty therapies. Qualifications

• PharmD degree and active or eligible Minnesota license.
• Experience in complex pharmacy operations or completion of an ASHP-accredited PGY1 residency.

Responsibilities

• Provide advanced clinical pharmacy services for oncology and specialty infusion patients.
• Act as a clinical resource for providers, nurses, and interdisciplinary teams.
• Ensure appropriate medication selection, dosing, and monitoring per guidelines.
• Oversee sterile compounding and hazardous medication verification per USP 797/800.
• Support oral oncolytic use through patient assessment and monitoring.
• Serve as a preceptor/mentor for students and residents.
• Participate in quality improvement projects and formulary management.
• Perform staff pharmacist duties to ensure operations across infusion sites.
• Adapt to schedule changes and site reassignment while maintaining continuity of care.
• Perform other duties as assigned.
• Shift length: 8 hours
• Weekend and holiday rotation; currently every 10th weekend

Education

Doctor of Pharmacy (PharmD) degree

1 year pharmacist experience in complex pharmacy operation or ASHP PGY1 residency

Job Overview

The Hematology/Oncology Pharmacist is based out of Southdale Cancer Center and is responsible for providing comprehensive clinical pharmacy services within a complex infusion pharmacy setting, with a focus on oncology and specialty infusion therapies.This role collaborates closely with an interdisciplinary care team to ensure safe, effective, and evidence-based medication use, supporting patient outcomes through medication therapy management, safety initiatives, and regulatory compliance.

This pharmacist serves as a clinical resource for patients, providers, pharmacy staff, and learners, while actively contributing to process improvement, formulary management, and departmental quality initiatives. The Clinical Pharmacist also participates in education and training for students, residents, and interdisciplinary care team members.

Job Expectations:

• Provide advanced clinical pharmacy services for oncology and specialty infusion patients, including daily order verification, therapy monitoring, medication adjustments, and documentation per department policies.
• Act as a key clinical resource, offering consultative support to providers, nurses, and interdisciplinary teams regarding pharmacotherapy, medication safety, and supportive care.
• Ensure appropriate medication selection, dosing, and monitoring in accordance with evidence-based guidelines and formulary protocols.
• Oversee sterile compounding and hazardous medication verification in compliance with USP <797>/<800> standards and institutional policies.
• Help ensure safe and efficacious oral oncolytic use through patient assessment and drug-specific monitoring.
• May serve as a preceptor and mentor for pharmacy students and residents, providing clinical education and training.
• Participate in quality improvement projects aimed at enhancing medication safety, workflow efficiency, and patient-centered care.
• Perform staff pharmacist duties as needed to ensure seamless pharmacy operations across assigned infusion sites.
• Adapt to schedule modifications and site reassignments as needed, ensuring continuity of care across all system infusion sites.
• Perform other duties as assigned.
• Shift length 8 hours
• Participation in weekend and holiday rotation and are currently every 10th weekend

Required Qualifications:

• Doctor of Pharmacy (PharmD) degree.
• 1 year pharmacist experience in complex pharmacy operation or completion of an ASHP-accredited PGY1 residency program.
• Licensed or eligible for licensure as a pharmacist in Minnesota

Preferred Qualifications

• Completion of an ASHP-accredited PGY2 Oncology Pharmacy Residency OR equivalent oncology practice experience.
• Doctorate of Pharmacy
• 3 years experience in a specialty setting
• Board certification (e.g., BCOP) is strongly preferred
