Job summary

Fairview Health Services is seeking a Pharmacist to deliver comprehensive medication management and clinical support in a complex hospital setting. The role partners with clinicians to enhance patient safety, education, and quality outcomes.

Responsibilities include direct patient care, SOP-based service delivery, and ongoing participation in regulatory and improvement activities. Minnesota licensure is required; ASHP PGY1 and 1 year experience are preferred.

Qualifications

• Minnesota Pharmacist License required.
• 1 year pharmacist experience in complex healthcare environment (preferred).
• ASHP-accredited PGY1 residency program (preferred).

Responsibilities

• Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary care team.
• Provide clinical pharmacy services to all patients in assigned area, including dispensing, reviewing, monitoring, and documenting changes per SOPs and regulations.
• Ensure appropriateness of medication orders, formulary correctness, and therapy in collaboration with the care team.
