Mercy South Sindelar Cancer Center in St. Louis, MO seeks a Registered Nurse (RN) Nurse Navigator to guide oncology patients through diagnosis, treatment, survivorship, and follow-up.

You will coordinate care with oncologists, infusion services, and inter-disciplinary teams to ensure timely, high-quality care and an outstanding patient experience. The role emphasizes education, barrier removal, and collaboration across departments to support patients and their families throughout their cancer

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2-3 years of nursing experience.
• Basic Life Support (BLS) certification.

Responsabilités

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effect management, and support resources.
• Facilitate communication between patients, providers, infusion services, imaging, and support teams.
• Develop and execute individualized care plans with physicians and interdisciplinary teams.
• Assist with referrals, appointment coordination, scheduling, and transitions of care.
• Monitor patient progress and address barriers to care proactively.
• Provide emotional support and connect patients with community resources and financial assistance.
