Job summary

Mercy South Sindelar Cancer Center in St. Louis, MO is seeking a compassionate Registered Nurse Nurse Navigator to guide oncology patients through diagnosis, treatment, survivorship, and follow-up care.

You will coordinate care with oncologists, infusion services, imaging, and support teams to ensure timely, high-quality care. You will educate patients and families about diagnoses, treatment plans, medications, and side effects; facilitate referrals and transitions of care; and document

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• 2-3 years of nursing experience.

Responsibilities

• Coordinate and navigate oncology care across diagnosis, treatment, recovery, and survivorship.
• Educate patients and families on diagnoses, treatment plans, medications, procedures, side effect management, and resources.
• Facilitate communication between patients, providers, infusion services, imaging, specialty providers, and support teams.
• Collaborate with physicians and teams to develop individualized care plans.
• Assist with referrals, appointment coordination, scheduling, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and connect patients with community resources.
• Maintain accurate documentation in EPIC and other clinical systems.
• Track patient outcomes to support quality metrics and program development.
• Serve as a clinical resource throughout the continuum of cancer care.
