Mercy South Sindelar Cancer Center in St. Louis, MO is seeking a Registered Nurse (RN) Nurse Navigator for the oncology program.

The role focuses on guiding patients through diagnosis, treatment, survivorship, and follow-up care, coordinating care with oncologists, infusion, imaging, and social work teams. Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2–3 years of nursing experience.
• BLS certification (required).
• BSN preferred.
• Oncology, infusion, hematology/oncology, or nurse navigator experience is a plus.
• OCN certification or willingness to obtain is preferred.
• ACLS certification is a plus.

Responsabilités

• Coordinate and navigate care for oncology patients throughout diagnosis, treatment, recovery, and survivorship.
• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, side effects, and support resources.
• Facilitate communication between patients, providers, infusion services, imaging, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor patient progress and address barriers to care.
• Provide emotional support and connect patients and families with community resources.
• Maintain accurate and timely documentation in EPIC and other clinical systems.
• Track patient outcomes and quality metrics to support evidence-based practice.
• Serve as a clinical resource for patients, families, providers, and staff.
