Mercy South Sindelar Cancer Center in St. Louis, MO, is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide oncology patients through diagnosis, treatment, survivorship, and follow-up care.

You will coordinate care with oncologists, infusion, imaging, and social work teams; educate patients and families; remove barriers to treatment, and document in EPIC to ensure high-quality care.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 2–3 years of nursing experience.
• Basic Life Support (BLS) certification.
• BSN preferred.
• Oncology/infusion/care coordination experience preferred.
• OCN certification preferred or willingness to obtain.
• ACLS certification preferred.
• Chemotherapy/biotherapy experience preferred.

Aufgaben

• Coordinate and navigate oncology care across diagnosis, treatment, survivorship, and follow-up.
• Educate patients and families about diagnoses, treatment plans, medications, procedures, side effects, and resources.
• Facilitate communication among patients, providers, infusion, imaging, specialty providers, and support teams.
• Collaborate with physicians and interdisciplinary teams to develop individualized care plans.
• Assist with referrals, appointment coordination, treatment scheduling, and transitions of care.
• Monitor progress and address barriers to care.
• Provide emotional support and connect patients with community resources.
• Maintain accurate documentation in EPIC and other systems.
• Track patient outcomes and quality metrics to support program development.
• Serve as a clinical resource for patients, families, providers, and staff.
