Job summary

Edward-Elmhurst Health Kellogg Cancer Center in Evanston seeks a Medical Social Worker to join the interdisciplinary team. You will perform psychosocial assessments, support patients and families, and coordinate resources to reduce barriers to cancer care.

Responsibilities include crisis intervention, care planning, education on community resources, referrals, and collaboration with oncology, palliative care, and care management teams.

Qualifications

• Master’s degree in social work from an accredited school
• Illinois licensure as LCSW or LCSW-eligible
• 2-3 years of social work experience in an acute hospital or health-care setting; oncology preferred
• Excellent interpersonal and organizational skills
• Strong written and verbal communication with professional documentation

Responsibilities

• Conduct initial psychosocial assessments to identify mood status, coping, support systems, and barriers to care
• Provide ongoing emotional support and short-term counseling to patients and families
• Educate patients and families on internal and community resources and coordinate connections to home care, transportation, grants, and support groups
• Collaborate with interdisciplinary teams (oncology, palliative care, IP care management) and coordinate referrals
• Document interventions in the patient’s clinical record in a timely manner
• Provide crisis management, safety assessment, and advocacy as needed
