Job summary

Illinois CancerCare is seeking a Licensed Clinical Social Worker to provide comprehensive psychosocial support to patients, survivors, and caregivers. The role is full-time, Monday–Friday, with no weekends or major holidays, emphasizing compassionate care within an oncology setting.

You will work on a collaborative team, conducting assessments, counseling, and care coordination, while integrating community resources and survivorship planning into patient care.

Qualifications

• Master’s degree in Social Work from an accredited program.
• Active clinical licensure in the state (LCSW/LCPC/LMFT preferred).
• Minimum 2 years of counseling, resource navigation, and case management experience.
• Experience with value-based care models (EOM) is a plus.

Responsibilities

• Provide emotional support, counseling, and crisis assessment for patients and families.
• Assess psychosocial impact of cancer across the care continuum and plan accordingly.
• Collaborate with the medical team to support patient and family needs.
• Participate in distress screening, advance care planning, and end‑of‑life conversations.
• Assist with survivorship transitions and connect to community resources.
• Develop and maintain community partnerships and patient education materials.
