Job summary

Illinois CancerCare is seeking a Licensed Clinical Social Worker to provide emotional, social, and practical support to patients, survivors, and caregivers throughout their cancer journey. This is a full-time, Monday–Friday position with no weekends or major holidays, offering strong work‑life balance.

Qualifications

• Master’s degree in social work required
• Current state social work licensure (LCSW, LCPC, or LMFT) preferred
• Minimum 2 years of professional counseling and case management experience
• Experience with value-based care models is a strong plus

Responsibilities

• Provide emotional support, counseling, and crisis assessment for patients and families.
• Support psychosocial needs across cancer care continuum, including diagnosis to survivorship.
• Collaborate with medical team and participate in care planning.
• Contribute to distress screening, end-of-life conversations, and transitions.
• Develop and maintain community resources referrals and patient advocacy.
• Document interventions in EMR accurately.
