Illinois CancerCare in Peoria, IL is seeking a compassionate Licensed Clinical Social Worker to join our supportive care team. This full-time role provides emotional, social, and practical support to patients, survivors, and caregivers throughout the cancer journey, with no weekend or major holiday work.

Qualifications

• Master's degree in social work from an accredited program.
• Current and valid clinical licensure in the state (LCSW preferred).
• Minimum of 2 years of professional counseling, resource navigation, and case management experience.
• Experience in value-based care models is a strong plus.

Responsibilities

• Provides emotional support to patients and families, including counseling and crisis assessment.
• Assists in transitions to survivorship and coordinates with community resources and programs.
• Works as part of an interdisciplinary medical team and supports distress screening and end-of-life discussions.
• Maintains and documents all interventions in the EMR accurately and timely.
• Develops and conducts support groups and educates clinic staff on psychosocial care.
