Responsibilities

• Accompany the physician in the exam room to document patient interactions.
• Document patient encounters in real-time using the EHR system with legible, clear notes.
• Chart patient history, exams, findings, treatments, diagnoses, and instructions.
• Prepare and present medical records for physician review and amendments before signing.
• Assist with chart rounds and coordinate with physicians and nursing staff as needed.
• Maintain confidentiality and comply with all relevant laws and departmental policies.
