Qualifications

• IV Certification strongly preferred.
• Certified Pharmacy Technician certificate or State Pharmacy Board requirement recommended.
• Experience in oncology pharmacy is desirable.

Responsibilities

• Prepare medication orders by compounding or admixing under supervision.
• Verify patient information and physician authorization for orders.
• Check insurance approval before admixing.
• Maintain accurate drug and lot number records and MSDS sheets.
• Prepare labels, file and process billing for medications.
• Ensure compliance with OSHA guidelines and department procedures.
