Qualifications

• High School diploma or equivalent.
• Entry level to three years experience in a pharmacy.
• Minimum three to five years experience in a pharmacy with I.V. skills.
• I.V. certification preferred.
• Certified Pharmacy Technician certificate or State Pharmacy Board requirement recommended.
• Minimum five to seven years experience in a pharmacy with I.V. skills.
• Oncology background preferred or extensive I.V. preparation experience.
• ASHP accredited program or community college based program recommended.

Responsibilities

• Assist with preparation of medication orders by compounding or admixing under physician supervision.
• Verify patient information and physician authorization for orders.
• Admixes drugs as ordered by physician in timely manner per guidelines.
• Calculate dosage volumes and convert units as needed.
• Maintain temperature logs and logs for protocol drugs.
• Support inventory control and label preparation; basic data entry.
• Clean and maintain admix area and uphold aseptic conditions.
