Fred Hutchinson Cancer Center in Seattle, WA is seeking a part-time Clinical Nurse Coordinator to support medical providers and manage care for an outpatient panel in Rehab Medicine. This 0.8 FTE role emphasizes nursing assessments, triage, education and care coordination within a multidisciplinary team.

The CNC will handle direct patient care, coordination with the care team, and documentation while enabling continuity of care for complex patients; a Washington RN license and BLS are required,

Qualifications

• Current BLS certification
• Oncology nursing experience preferred
• Strong clinical judgment and decision-making

Responsibilities

• Assess patient needs across physical, emotional, social, and spiritual domains
• Coordinate with interdisciplinary team for continuity of care
• Educate patients and families on disease processes and treatments
• Manage care plans and referrals within Rehab Medicine
• Document nursing activities accurately in the EMR
