Job summary

Ochsner Health in New Orleans seeks a RN Discharge Navigator for oncology and blood and marrow transplant patients transitioning from inpatient to outpatient care. You will lead discharge planning, educate patients and coordinate with a multidisciplinary team to ensure safe transitions and timely follow-up.

Strong communication, clinical nursing experience in oncology and BMT, and flexibility for a hybrid schedule are essential.

Qualifications

• Requires graduate of an accredited nursing program.
• RN license in state of practice (required).
• 2 years of nursing experience (required).
• BLS from American Heart Association (required).
• Oncology, hematology/BMT, discharge planning or navigation experience preferred.

Responsibilities

• Serve as primary discharge navigator for hospitalized oncology and BMT patients.
• Conduct nursing assessments, identify discharge barriers, provide education on hospitalization, discharge and follow-up care.
• Coordinate multidisciplinary discharge planning with physicians, nursing staff, pharmacists, social work and care management.
• Facilitate timely discharge planning, addressing barriers that delay discharge or impact recovery.
• Schedule follow-up appointments and post-discharge services; triage patient concerns with providers.
• Reinforce cancer treatment and transplant care education, infection prevention and escalation pathways.
• Maintain accurate EMR documentation of assessments, interventions and outcomes.
• Collaborate in discharge planning meetings and ongoing care coordination to optimize outcomes.
