Job summary

Anchor Health is seeking a Hospice RN Case Manager to coordinate and deliver end-of-life care in patients' homes and communities. The role requires CA RN licensure and CPR certification, with hospice experience preferred. You will work within an interdisciplinary team to manage care plans, assess needs, and educate patients and families.

Qualifications

• RN license valid in CA(Required).
• Hospice experience preferred (1 year).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and update the hospice care plan for assigned patients.
• Provide pain and symptom management and education to patients and families.
• Complete initial and ongoing assessments and document care.
• Collaborate with the interdisciplinary team to meet patient needs.
• Maintain accurate clinical notes and report changes to physicians.
