Job summary

Anchor Health is seeking an experienced Hospice Team Nurse to plan, organize and direct patient care in the home. You will collaborate with physicians and families, and coordinate care with the interdisciplinary team to ensure compassionate, high-quality service.

The role emphasizes excellent observation and communication, with requirements including CA RN licensure, CPR, and a preference for 2+ years in hospice, home care, or public health. A sign-on bonus may apply.

Qualifications

• Graduate of an NLN-accredited nursing school.
• Current CA RN licensure and CPR certification.
• 1+ year professional nursing experience within the last 3 years or a BSN with RN license.
• Preferred: 2+ years in public health, home care, or hospice.
• Excellent observation and communication skills; basic math skills.

Responsibilities

• Plan, organize and direct hospice nursing care in the home.
• Communicate patient needs to the Interdisciplinary team.
• Coordinate patient care with physicians and family.
• Acts as Case Manager when assigned by Clinical Supervisor.
• Maintain accurate clinical notes and report changes in patient status.
