Job summary

Anchor Health is seeking an experienced hospice RN Case Manager to join our team in California. You will conduct initial and ongoing assessments, perform home visits, and develop caregiver-inclusive care plans to meet hospice needs.

Responsibilities include administering medications, participating in interdisciplinary meetings, documenting patient status, and supervising ancillary staff as required. Candidates must hold a CA RN license, CPR, and a valid CA driver's license with insured vehicle,

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR Certificate.
• Minimum of two years experience, at least one in public, home care, or hospice nursing, or a BSN with RN license and required experience.
• Must have a valid CA driver license and an automobile insured as required in CA.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Performs home visits and assesses patient status.
• Implements and revises the plan of care based on patient needs.
• Develops care plans with patient and family, incorporating nursing actions.
• Administers medications and treatments as prescribed by the physician.
• Attends Interdisciplinary Group Meetings and documents care.
• Supervises ancillary personnel when required.
