Job summary

Hope Solutions in Richmond, CA is seeking a dedicated Case Manager to deliver on-site services for seniors residing in an apartment complex. You will assess needs, facilitate access to medical, behavioral health and legal services, and support housing stability through eviction prevention.

You will collaborate with the Housing Authority and health service staff to develop and monitor individualized service plans and assist with documentation, referrals, and follow-up visits.

Qualifications

• BA degree in social work or related field; equivalent experience will be considered.
• Minimum of two years’ experience in the human service field, preferably with housing retention barriers such as substance abuse, mental health, or income challenges.
