Case Manager
We are seeking a Case Manager to deliver on‑site case management services to seniors residing in an apartment complex in Richmond, CA. The role involves ongoing assessment of individual needs, facilitating access to medical, mental health, addiction, financial, and legal services, and supporting housing stability through eviction prevention and landlord/management relationships. The Case Manager works collaboratively with the Housing Authority, Health Housing and Homeless Services staff, and other providers.
Responsibilities
- Maintain awareness of culturally diverse consumer populations and perform duties with attention to cultural needs.
- Maintain contact with all consumers in the caseload and provide home visits according to an identified level of need.
- Perform comprehensive psychosocial, life skills, and financial assessments.
- Work collaboratively with consumers to develop and monitor progress on individual or family service plans.
- Provide case management support, including budgeting assistance, accessing needed benefits, and accompanying consumers to medical or mental health appointments.
- Ensure provision of supportive mental health interventions as needed, including crisis management.
- Link consumers to medical, mental health, addiction/recovery, financial, legal, and other community services.
- Assist consumers in locating units with landlords or property managers who accept voucher payments or explore lease negotiation with current landlords.
- Collaborate with all providers to overcome eviction risks related to behavioral, mental health, or physical limitations.
- Assist with consumer certification and verification paperwork.
Organizational/Administrative Responsibilities
- Represent Hope Solutions professionally in all circumstances.
- Comply with all legal and ethical guidelines for maintaining consumer confidentiality, HIPAA, consumer rights, and safety.
- Comply with local, state, and federal regulations and Hope Solutions policies.
- Complete required administrative paperwork and reports accurately and timely.
- Attend required meetings, including staff, team, consumer case conferences, and agency linkage meetings.
- Complete personnel paperwork and expense reports for food, petty cash, and mileage promptly.
- Document all services in a timely manner.
- Assist with data generation for reports and funding applications.
- Perform other duties as assigned.
- Be available for on‑call, after‑hours, or backup work.
Work Location Classification
- Site‑Based: Work is primarily performed at an assigned Hope Solutions location. On‑site presence is required for administrative, program, and client‑support activities.
- Work from home requires prior approval by the Program Director or Manager via written approval.
- New program employees must work on‑site for the first 90 calendar days; exceptions require approval.
- Client‑facing work may occur on‑site or in the field, including appointments, sessions, and support services.
- Expectations may flex to accommodate operational needs, client availability, safety concerns, or urgent program priorities while meeting performance expectations.
- Work‑from‑home requests must include requested dates/hours, business reasons, and a work plan. Urgent same‑day requests should be notified to leadership as soon as possible.
- When approved to work from home, employees must maintain confidentiality, work from a professional private environment, complete tasks on time, and track time accurately.

Position Qualifications
- Knowledge of and belief in a Housing First philosophy and strategies.
- Demonstrated experience supporting vulnerable populations, especially homeless clients with mental disabilities.
- BA degree in social work or related field; equivalent experience will be considered.
- Minimum of two years’ experience in the human service field, preferably with housing retention barriers such as substance abuse, mental health, or income challenges.
- Bi‑lingual in English/Spanish preferred.
- Outstanding written and verbal communication skills.
- Computer proficiency with Microsoft and database applications.
- Must have an operational vehicle, auto insurance, valid California driver’s license, and be willing to drive for work.
- Must pass LiveScan screening and TB test.
- Salary contingent upon skill and experience.
Physical Qualifications
- Must be able to climb stairs to consumers who live on second or third‑floor apartments without elevators.
- May accompany consumers on walks up to half a mile.
- Must be able to work at a desk for up to 3 hours at a time for calls, research, and collaboration.
- Office tasks include typing, entering case notes into a database, and copying/faxing documents.
- Occasionally lift up to 25 pounds, including furnishings, files, or food donations.
- Ability to drive an automobile and transport consumers to appointments, including night driving.
