Job summary

Senior Services of Southeastern Virginia is seeking a Housing Stabilization Case Manager to provide housing-focused support for older adults facing instability. Duties include assessing needs, coordinating services, and monitoring progress to achieve housing stability.

The role involves crisis counseling, linking to medical care, and overcoming barriers with trauma-informed, culturally appropriate approaches.

Qualifications

Bachelor’s degree in social work, human services, or related field is required or equivalent combination of education and experience.
At least one year of experience in community-based services is required.
Excellent customer service and telephonic communication skills.
Must possess a valid driver’s license, auto insurance, a good driving record, and pass an acceptable criminal background check.

Responsibilities

Facilitate person-centered, problem-solving conversations to identify participant needs and goals.
Provide housing-focused support for older adult program participants.
Complete housing barriers assessments and develop individualized service and housing plans.
Coordinate referrals to other providers and support participants in following through with referrals.
Monitor and evaluate participants’ progress toward goals.
Collect data in HMIS and other platforms.
Support community outreach and program evaluation as needed.
Collaborate with internal teams to ensure holistic support and smooth operations.
Perform other duties as assigned by supervisor to support the program mission.
