AmeriPharma is hiring a Patient Care Coordinator I for an on-site role in Laguna Hills, CA. You will be the primary contact for patients and caregivers, coordinating deliveries and collecting essential information while ensuring regulatory compliance.

Qualifications

• At least one year of customer service experience, preferably in healthcare.
• Excellent verbal and written communication skills.
• Fluency in English, including speaking, reading, and writing.
• Proficiency in Microsoft Word, Excel, Outlook.
• Typing speed of at least 30 words per minute.
• Strong organizational skills and attention to detail.
• Ability to manage multiple routine tasks in a fast-paced environment.
• Ability to follow established procedures and accept feedback.
• Ability to work independently within routine workflows.
• Self-motivated and collaborative.

Responsibilities

• Provide professional customer service through inbound and outbound communication with patients, caregivers, nurses, physician offices, and internal departments.
• Perform routine patient coordination activities in accordance with established departmental workflows and procedures.
• Review patient profiles, delivery schedules, medication storage instructions, and new patient admission information for accuracy and completeness before confirming deliveries.
• Schedule timely medication deliveries for AIC and home infusion patients, maintaining a scheduling window of at least 14 days whenever possible.
• Communicate and document supply coordination requests received from patients, caregivers, and nurses.
• Collect copayments through approved payment methods as needed.
• Coordinate follow-up with pharmacists, nurses, or other departments as required.
• Ensure patients are connected with a pharmacist for medication consultation when required by law.
• Maintain HIPAA and regulatory compliance in all communications.
• Maintain complete, accurate documentation within pharmacy operating systems.
• Support training, quality improvement activities, and implementation of approved technology.
• Demonstrate accountability, professionalism, reliability, collaboration, and a patient-centered approach.
