Job summary

Circadia Health in Los Altos, CA is seeking a licensed Nurse Practitioner to join our on‑site team. You will provide direct patient care, respond to predictive health signals, and coach facility staff to integrate Circadia’s technology into daily clinical workflows.

This full‑time role requires Monday–Friday on‑site presence at our partner facility, with a mix of assessments, care planning, documentation, and collaboration with clinical ops and product teams to reduce readmissions.

Qualifications

• Certified and licensed Nurse Practitioner in CA.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; capable of independent practice.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English proficiency required.

Responsibilities

• Direct patient care: initial and follow-up assessments on flagged residents; initiate/adjust plans of care; coordinate with facility providers.
• Tech → bedside integration: train staff and standardize Circadia into clinical workflows.
• Data into action: interpret trend‑based insights, prioritize high‑risk patients, drive timely interventions.
• Close the loop: complete documentation within the same day.
• High‑volume rounding: conduct 30+ visits/day across SNFs when appropriate.
• Quality & product feedback: surface field insights to improve workflows and tech integration.
• Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
